Hazardous Materials
The Hazardous Materials form is required for all stocked products that are
restricted by DOT (Department of Transportation) and needs to be updated and submitted to
the product manager whenever there is a change. This form is also required on all
new products being proposed to United Stationers that carry a DOT restriction at the time
of catalog review.

and Shipping Requirements
The Manufacturers' Packaging, Labeling and Shipping Requirements brochure
includes detailed instructions on United Stationers' requirements for carton labeling,
shipping documentation and other important performance criteria to consider when making a
shipment to a United Stationers distribution facility.

Inbound Delivery Request Form
This excel file is used to request delivery appointments via email
for all United Stationers Regional Distribution Centers. The
Inbound Delivery Request form must be filled out complete and
accurately in accordance with the Packaging, Labeling and Shipping
Requirements guide.
Routing Guide and Bill of Lading Requirements
The Routing Guide and Bill of Lading instructions are intended as a general guideline for
standard inbound shipping to a United Stationers facility, or a drop shipment NL
(non-listed) order to a United Stationers customer.
